Our assessment process begins with an initial consultation with family members, either in person or by telephone. We gather essential data including a full family and medical history prior to meeting with the elder. We will then visit the elder at home, in a facility, or at a family member's house to conduct an in-depth evaluation of their physical, cognitive, and emotional health. Using the insights we've gained from the family history, we take the time to get to know our clients and help them feel comfortable by establishing an honest and trusting relationship.

Current medications, mobility issues, confusion, memory loss, depression, anxiety, fears, and isolation are addressed. Spiritual needs by visiting pastoral care, social engagement, and community activities with transportation services.

Physical and cognitive impairment affect safety at home. Are medications being taken appropriately? Is driving a concern? Potential hazards include expired medications & foods. Is the home in a safe condition to live in?
Elder protection and abuse prevention are evaluated with ongoing visits.

A realistic care plan for services and alternative living options include assets and income evaluation. Advanced Directives, Living Wills, Health Care Proxy, Power of Attorney, and Legal are addressed. Home health, Meals, medication management.
